uhh I think I'm not sticking out my butt enough. it leads to me "dive bombing" as David would call it, and basically having not as good balance and control on the way back up consistently.
That's why I failed on rep 2 of that first attempt of the third working set
I'm going to gradually improve my form on lower weights and hope that it catches up
